What to Expect at First Visit

For new patients who schedule appointments ahead, our staff will either send you out a welcome packet of clinic information or send you an appointment reminder postcard if you choose to download new patient forms from our website.

Please arrive 15 minutes prior to your appointment time and bring your new patient forms, insurance card, any prior health records, and copayment or deposit (if applicable).

The Family Wellness Center participates in many health insurance plans and networks. If you have health insurance, please alert our front office and provide us with your card so that we can scan it into your electronic record. It is always the patient's responsibility to know what their health insurance is, if there is a copay or coinsurance, and what coverage restrictions may apply.

Information about the estimated charges of your health services is available upon request from our billing department. Please do not hesitate to ask for information. If you have any questions as to whether Family Wellness Center providers are preferred providers with your insurance network, please call your health plan to inquire.
