Medical Records

Family Wellness Center has an office assistant available to serve you in our medical records department Monday thru Friday 8:00 am to 4:00 pm. All FWC patient records from 2007 to current are available for a fee in either paper format or electronic copy on a CD (in Adobe Acrobat pdf file format). Please call our records department if we can assist you with acquiring a copy of your records at: (360) 567-0326.

Unless urgent, please allow 7 - 10 working days for your paper format records and 2 working days for your electronic format records. Thank you.
