Chronic Pain Management

On January 2, 2012 the Washington State Medical Quality Assurance Commission passed new rules for the management of chronic noncancer pain.   The 2010 Legislature passed a bill directing five boards and commissions to adopt rules concerning management of chronic, noncancer pain (CNCP).  CNCP is generally defined as pain present for at least 6 months; lasting longer than the expected time to tissue healing or resolution of the disease process; or pain due to a condition where there is ongoing pain perception or from a damaged nerve.

The intent of the bill is to equip physicians with the best practices in pain management and keep patients safe.  Nothing should change for the majority of patients.  Under certain circumstances, our primary care providers may seek consultation from a pain specialist.  The pain specialist would not care for the patient on a regular basis, only provide consultation.

The role of the patient is to see their primary care physician with reasonable expectations.  The patient and the PCP will need to come up with a comprehensive treatment plan for the patient and for the pain, with clearly defined goals for everyday functioning that can be reasonably met.  Prescription pain medications (controlled substances) should only be part of a treatment plan and exercise should be a part of the treatment plan as well.  It is the patient's responsibility to keep these medications secure in the home and while the patient travels to avoid unintentional poisoning.
